2015回饋報告:不同性別之戒菸者二階段戒菸諮詢架構教育訓練方案(第二年)

摘要

台灣戒菸專線自2003年開辦以來,協助許多吸菸民眾戒菸。而以近年來的服務成效來看,多次諮詢戒菸成功率均達30.0%以上,且整體滿意程度達90.0%以上。專線成立之初得到美國加州戒菸專線的技術指導,並以加州戒菸專線模式為主要的學習對象。雖有前人經驗做為借鏡,但實際在運作上,也因國情、環境、文化、資源及政策等面向之不同,在實務工作中有著許多的摸索與調整,而所有的努力,均在於希望能夠建構一個符合國情、具成效、且能夠與國際接軌的本土戒菸諮詢模式。

專線的諮詢模式,由於聚焦於戒菸與促進健康行為之議題,並且是行動取向,加上所服務的人數眾多,故所提供的服務為短期諮詢。專線於97~99年建立的二階段戒菸諮詢模式,已提供了更為貼近理論及實務的方針;100~102年亦著重此諮詢架構,而建立了ㄧ套具系統性的在職訓練方案,提升了戒菸諮詢服務的品質及成效。然而,由103年「評估與瞭解不同性別之戒菸者其戒菸經驗」研究發現,不同性別的戒菸者,於戒菸歷程中所涉及「心理」、「生理」及「社會環境」的困難與諮詢人員之服務策略,有其共通與差異之處。因此,本計畫之主要宗旨乃在於設計與評估不同性別的吸菸者二階段戒菸諮詢架構。

今年度為三年計畫的第二年,研究目標在於依據103年之研究,發展「不同性別之戒菸者二階段戒菸諮架構」之教育訓練方案,以協助諮詢人員更能夠在面對不同性別之戒菸者,提升其服務順暢度並增加戒菸諮詢的專業能力,讓不同性別之戒菸者獲得更符合需求的諮詢介入,課程或個案研討包含情緒與壓力管理、人際社交技巧訓練及睡眠衛教等。由本回饋報告來看,課程設計的安排已趨於完善。專任諮詢員建議後續之課程規劃,除了延續此課程議題外,應可以工作坊、督導團體或讀書會等型式,增加進階課程的多元性,以及深度探討性別對戒菸諮詢的影響,期再加強兼任諮詢員之專業內涵。

ABSTRACT

In the initial stage, Taiwan Smokers’ Helpline (TSH) adopted a successful smoking cessation quitline model from California Smokers’ Helpline. Over the past 11 years, Taiwan Smokers’ Helpline has been adjusted this model according to the culture, resources, and policies in Taiwan and developed its own smoking cessation telephone counseling protocol. Recent TSH survey demonstrates that quit rate of clients received our multi-sessional counseling is above 30% and overall client satisfaction is more than 90.0%.

Service provided by Taiwan Smoker’s Helpline is using action-oriented approach to help smokers increase their health-promoting behaviors. TSH established a preliminary telephone counseling protocol during 2008 and 2010 and built a serial structured training program during 2011-2013. In doing so, service quality and efficiency has been improved. However, in our 2014 report, we noticed there were gender differences in difficulties and strategies during quitting process. Therefore, it is necessary to develop telephone counseling protocols for male and female smokers accordingly.

This study is the second part of the research project “Serial Studies on Developing Two-Stage Smoking Cessation Counseling Protocols for Male and Female Smokers”. We hope to construct a training program according to the needs and difficulties of male and female smokers to help our counselors to be more capable in providing service and intervention. The topics of the training program include, “emotion and stress management,” “interpersonal skills,” “sleep hygiene,” and so on. Feedbacks from the participating counselors about the design of the training were positive. They also suggested and expected to have more advanced courses in various forms, such as workshop, group supervision, and study group.
