2016回饋報告:不同性別之戒菸者二階段戒菸諮詢架構教育訓練方案 (第三年)

摘要

戒菸專線自103年開始,針對戒菸之性別議題進行研究,希望瞭解不同性別戒菸民眾於戒菸歷程中的正、負向影響因子,以規劃更細緻的戒菸諮詢工作任務與服務架構,使不同性別戒菸者獲得更符合需求的服務而順利戒菸,並提升專線之諮詢品質與內涵。104年進而發展「不同性別之戒菸者二階段戒菸諮詢架構」之教育訓練方案,以協助諮詢人員增加對不同性別之戒菸者的專業能力。本研究為三年計畫之第三年,主要依據103年之研究結果,設計「不同性別之戒菸者二階段戒菸諮詢架構」之介入能力評量表,並以104年所建立之訓練模式基礎,檢視105年規劃之「不同性別之戒菸者二階段戒菸諮詢架構」教育訓練方案執行成效,以瞭解諮詢人員在課程中的學習有效度、熟練度及課程滿意度,及在職訓練是否提昇諮詢人員面對不同性別戒菸者的工作成效。

本研究以線上服務狀況調查表,蒐集諮詢人員在面對不同性別戒菸者之介入能力的前後測評量資料,瞭解諮詢人員之專業能力,並輔以在職訓練課程回饋表與質性學習心得,評估諮詢人員之學習狀況,再加上外部專家評鑑和103-105年一個月點戒菸率之客觀服務評量指標,檢視專線工作之組織有效性。

本研究結果與建議分述如下:(1)經「不同性別二階段戒菸諮詢架構」訓練課程後,整體諮詢人員之介入能力有顯著提升,學習有效度及熟練度值得肯定。(2)本年度在職訓練課程,使諮詢人員對不同性別戒菸者之敏感度、介入能力,以及諮詢服務專業能力皆有所提升。(3)外部評鑑專家認為專線諮詢人員對男性戒菸者之服務效能皆在可接受以上之程度,但對於女性戒菸者之服務效能則還需努力。(4)對照專線103-105年度之一個月點戒菸率來看,接受專線服務之戒菸者整體戒菸率已有所提升,特別是104年度女性戒菸者之戒菸率突破四成,而男性戒菸者之戒菸率則維持在三成多左右。建議未來專線可針對儲備訓練與在職訓練建立較具系統性與一致性之課程,並再增加助人與諮商技巧之專業內容,提升諮詢人員服務不同類型案主的能力,在訓練方式上也可參考過去研究建議,採用多元的方法增進諮詢人員之學習效率。而在研究方向上,專線內部對服務效能之調查與來電個案特性之分析需更加細緻,除性別議題外,個案動機、年齡、需求等變項,值得未來繼續進行探索與研究。



Serial Studies on Developing Two-Stage Smoking Cessation Counseling Protocols for Male and Female Smokers (3rd Year Report)—
A Preliminary Model of Two-Stage Smoking Cessation Counseling Protocols for Male and Female Smokers

ABSTRACT

This study is a part of a three-year research project started in 2014. The project is aiming to develop a telephone smoking cessation counseling protocol by considering gender differences. In the first year of the research, we tried to understand positive and negative factors of quitting smoking for male and female smokers’. In the second year, we hoped to construct a training program according to the needs and difficulties of male and female smokers in order to help our counselors to be more capable in providing service and intervention.

The purpose of this study was to assess the effectiveness of the training program developed in 2015. Counselors participated in this study were asked to completed a pretest and a posttest regarding to their capability of smoking cessation interventions for male and female clients. Participants’ feedbacks of the training, external review of quality of service and 7-day point abstinent prevalence rate at one month after quit day were also collected for analysis.

The results demonstrated significant improvement of counselor’s sensitivity to gender differences in callers’ needs and difficulties during smoking cessation. The training program also enhanced counselor’s ability of making intervention by taking gender differences into account. External review shows greater effectiveness of the counselors’ while helping male callers. However, 7-day point abstinent prevalence rate of male callers in year 2014 to 2016 is around 30% and that of female callers is more than 40% in year 2015.

According to this study, a more coordinated and systematic design of pre-job and on-the-job training is suggested. In addition, the proportion of counseling skills in training should be increased. The relationships among caller’s demographic characteristics alone with their motivation to quit smoking and their needs of assistance are worth further investigations.
