2017回饋報告:形塑與建立不同年齡層之戒菸者二階段戒菸諮詢架構 【第一年】 不同年齡層之戒菸者戒菸困難、動機以及吸菸認同對戒菸行為的影響

摘要

戒菸專線於103-105年之間,針對戒菸之性別議題進行研究,希望瞭解不同性別戒菸民眾於戒菸歷程中的正、負向影響因子,以規劃更細緻的戒菸諮詢工作任務與服務架構,使不同性別戒菸者獲得更符合需求的服務而順利戒菸,並提升專線之諮詢品質與內涵。106年至108年則針對不同年齡層之戒菸者進行研究,從戒菸成功因素、失敗因素、所需協助及相關議題進行探討,期待藉此規劃符合不同年齡層戒菸諮詢架構,使線上諮詢人員未來在協助不同年齡吸菸者時,更能將符合其需求,使戒菸諮詢工具更具效能。

因考量專線電訪調查工作安排,本研究區分為兩個時段進行資料收集,第一階段問卷收集時間為104年1月至9月份進線案主,主要探討不同年齡層戒菸者戒菸成功及戒菸失敗因素,並了解不同年齡層對專線工作的期待,共收案人1,846人(其中戒菸成功645人,戒菸失敗1,201人)。第二階段問卷則於106年4月1日開始進行,收集至106年8月31日,主要探討戒菸認同對戒菸成效的影響,共收案222人。

本研究結果與建議分述如下:(1)不同年齡層的吸菸者其戒菸成功的因素有相同亦有相異之處。各年齡層均提及意志力對戒菸的重要性。然而意志力為較模糊定義,未能轉化為有效的介入策略,未來研究時,應將意志力一詞形成操作型定義,以能回應至未來的諮詢實務工作。(2)排除相同因素,青少年族群及老人族群認為家人/朋友的支持為其能戒菸重要的影響因子。然而在壯年及中年族群中,戒菸藥物的功能則是其認為重要的助力。上述不同可能來自不同時期的成癮狀況以及生活重心,對青少年來說,尼古丁成癮狀況較低,不建議使用戒菸藥物等因素,降低其對藥物需求的反應,而壯年及中年時期,成癮狀況較嚴重並且仍在家庭與事業的巔峰年齡,令人不適的戒斷症狀需要藥物緩解,可能使其肯定戒菸藥物對於戒菸歷程的幫忙。(3)在吸菸認同對戒菸結果影響的調查中發現,「菸在生活中是重要的嗎?」在第一個月及第三個調查的結果有較明顯的差異,若依據戒菸者狀況有無改變來分組,不同組別呈現的狀況有異。依據戒菸者狀況有無改變來分組,不同組別呈現的狀況有異,其中一個月尚未戒菸但在三個月調查時已戒斷的組別中可以發現,吸菸認同會隨戒菸結果有所改變,與均未戒菸組別相較下,此組別的吸菸認同在一開始亦有差異,對吸菸的看重程度低於均未戒菸的組別。
 

 

Serial Studies on Developing Two-Stage Smoking Cessation Counseling Protocols for Smokers of Different Age Groups (1st Year Report) –The Impact of Obstacles, Motivation, and Smoker Identity on Smoking Cessation Behaviors of Smokers in Different Age Groups

ABSTRACT

During 2014-2016, we conducted research on understanding positive and negative factors that affected male and female smokers in their smoking cessation process. In the year 2017 to 2019, we look into various elements that influence smokers in different age groups and to tailor our telephone smoking cessation counseling protocols to meet their needs.

We used two sets of data for analysis. One set of data were collected from 1846 smokers in various age groups who received our services during January to September 2015; among those, 645 succeed and 1201 failed to quit smoking. We sorted out the positive and negative factors related to success and failure in smoking cessation. We also outlined the expectations of smokers to the quitline in different age groups. The other set of data were collected from 222 clients during April 1st to August 31st, 2017 and analyzed for the impacts of smoker identity on the outcome of smoking cessation.

Results and suggestions are described as follows: (1) Smokers in all age groups viewed willpower as a key factor leading to the success of quitting. However, willpower needs to be operationally defined in order to reflect on intervention. (2) During cessation process, supports from family and friends were crucial to the adolescent and the elderly smokers, whereas pharmacotherapy was considered essential to smokers between 20-64 years of age. These age variance may attribute to the different degree of physical addition to nicotine and the differences in centers of life. (3) When interviewed in the third months after receiving cessation counseling, those who did not make any progress on quitting smoking considered smoking was more important in the daily life than any other groups. For those who had quitted smoking in the third months after receiving cessation counseling, smoking became less and less important to them.
